Enhanced Care Management


There is a new Medi-Cal benefit called “Enhanced Care Management (ECM)”. ECM is one of several improvements within the California Advancing and Innovating Medi-Cal initiative (CalAIM). ECM provides extra services at no cost to Medi-Cal members who qualify. ECM providers conduct outreach to assigned health plan members, engage them in enrolling into ECM, and deliver ECM services!

Hope Services’ Counseling Center provides mental health services for adults and children, ages five and up, with developmental disabilities and co-occurring mental health needs. A multi-disciplinary team provides all services, including Enhanced Care Management.

We currently accept referrals from Santa Clara Family Health Plan (SCFHP) for ECM services.
