- Job Description:
The RN will work collaboratively with psychiatric medical staff in relationship to the physical and mental health needs of the clients. The nurse performs medication support evaluations and services and assists medical staff in medication appointments. She works within the scope of her license in assisting clients with their psychiatric and physical health medications. Performs patient and caregiver education regarding efficacy, side effects and dispensing of medication. She does not prescribe medications but may at the physician’s request assist with renewals at MD direction.
Assures patient’s physical health needs (ie diabetes, hypertension, seizures) are being addressed and obtains signed releases of information and talk with primary care and specialty physicians (neurologists, cardiologists) to coordinate their physical health care and mental health care. Is responsible for preparing materials for County Mental Health audits, and follow-up.
Will obtain vital signs, including blood pressure, body mass index, UTS on regular basis for all clients. Will perform an initial physical health status inventory and obtain appropriate past medical records. Assists clients with comorbid medical conditions.
Will lead time-limited problem-specific groups around diabetes, nutrition and weight, cardiovascular and neurological conditions.
Assists clients in obtaining appropriate adjunctive tests including blood tests and procedures and assures medical specialty appointments.
Works with clients concerning side effects of psychiatric medications and provides patient and caregiver information regarding medications being used. Takes a lead in
liaising with medical institutions, hospital system, pharmacies and San Andreas Regional Center.
To improve the quality of life for people with developmental disabilities.
Assess efficacy of psychiatric medications and side effects.
Liaison with physicians and community doctors and nurses in regard to psychiatric care and management of psych meds and physical medicine needs of clients.
Conduct basic assessment of the individuals. Assist physicians and other providers in delivery of care, especially medication changes..
Discriminate between normal conditions and normal and abnormal deviations by specific screening and tests. Refer patients with abnormal findings to physicians. Link clients for laboratory work and review findings in conjunction with psychiatrists.
Obtain treatment consents as needed. Assist in obtaining TARS and JV 220’s.
Responsible for County physician audits quarterly.
Assist in linking clients with their medications and with primary care.
Provide counseling and health training.
Maintain accurate records.
Participate in research and development projects.
Perform related duties based on departmental need.
Registered Nurse. Current/valid RN license in the state of California. Experience in psychiatric nursing setting.

CPR and First Aid certification. Must be able to respond to first aid injuries, and perform Heimlich and CPR when alone with consumers in the community.
Positive and cooperative attitude toward a consumer driven service system working with severely mentally ill adults with developmental disabilities.
Ability to be flexible with scheduling and work in a consumer driven environment
Strong interpersonal skills. Ability to work with a diverse population.
