Community Living Services 

Community Living Services, as are all of HOPE's other programs, are designed to assist people with a wide range developmental disabilities to fully participate in the community.  

Who can participate in HOPE’s Community Living Services? 

HOPE's Community Living Services are available for any person with a developmental disability who: 

  • Is 18 years or older and a client of a regional center.

  • Lives in geographical area that is being served. 

What Services are available?  How does this program work?

Supported Living Program

Supported Living Services (SLS) provides a range of services and supports to persons with developmental disabilities to enable them to accomplish their dream of living in and having a valued role in the community.  Persons in HOPE’s SLS program are supported in living arrangements that are typical for persons without disabilities (e.g. apartments, condominiums and houses).  They have control over their living arrangements by being the leaseholder or owner.  All decisions, plans and actions begin with the needs and wants of the client.  Individuals receive the supports they need to succeed, such as assistance with daily living activities, paying bills, furnishing their home, accessing the community, and interfacing with other agencies. Emergency support is available 24 hours a day, 365 days a year.

Independent Living Skills Program

The goal of HOPE’s Independent Living Skills (ILS) program is to provide training to a person who may currently be or is preparing to live unsupervised in a residence of their choice.  The service begins with a comprehensive assessment of current independent living skills.  A training plan is then developed to strengthen an individual’s ability to live successfully in an independent environment.  Training areas may include financial management, health and safety needs, mobility training, personal and social skills, recreation and leisure skills, and accessing community resources.  Services end when the individual has met their goals. 

Who provides the services? 

Community Living Services employs ILS trainers to provide one on one direct services to individuals.  Staff are trained by HOPE Services to perform all related job duties and are experienced, open and sensitive to meeting the needs of people with disabilities, able to ascertain the needs of a person with disabilities and recommend services that benefit and enhance the quality and life of that individual, as well as being knowledgeable about how to access community services.   Staff and clients are matched by compatibility and clients have choice of which staff they want working with them.  All employees go through a background screening. 

Supported Living Services employs support specialists (case managers), companions (direct care daytime staff), and roommates (direct care overnight staff)

 

Independent Living Services features skills trainers who are direct service staff in the teaching role with clients.

 

What Accommodations are available? 

HOPE’s Programs and Services are designed to support individuals with disabilities in their communities and activities.  HOPE facilities and programs follow Americans With Disabilities guidelines for accessibility for full inclusion of individuals who utilize services and participate in programs.  All efforts are made to accommodate individual’s physical, personal care, communication, and behavior support needs.   

How can you enroll in these services?

If you receive services through a Regional Center, such as San Andreas Regional Center (SARC) or Golden Gate Regional Center  and want to participate in Community Based Day Services, you can speak with a Service Coordinator (Worker, Social Worker) who can authorize HOPE to provide this services to you or your family member. 

Where are HOPE’s Community Living Services offered? 

Independent Living Services and/or Supported Living Services are offered in all five districts of HOPE Services.
